New to the practice? Start here.
Becoming a patient at Mountain Dental starts with a short form and a phone call. Fill out our new patient form and our new patient coordinator will call you back to answer your questions and find a time that works. We see kids, teens, and adults.
How it works.
The form takes a few minutes and tells us what we need to get you to the right first visit. Once you submit it, our front office team reviews it and calls you within two business days. That call is where we answer your questions, confirm your insurance, and book the appointment that fits what you need. We keep this part personal on purpose.
Your first visit takes a little longer, and that’s the point.
A new patient visit is a full comprehensive exam, not a quick look. We take the time to understand your history, look closely at what’s going on, review your x-rays with you, and talk through what we find and what your options are.
Before your visit.
We’ll already have your insurance details from your call, so there’s nothing to bring. Just plan to arrive 10 to 15 minutes early to complete any new patient paperwork before your appointment time.
